What Is IASTM?

Instrument-Assisted Soft Tissue Mobilization (IASTM) is a skilled manual therapy technique that uses specially designed stainless steel or composite instruments to detect and treat soft tissue dysfunction. Often referred to as “muscle scraping,” IASTM allows clinicians to effectively break down fascial restrictions, scar tissue adhesions, and areas of chronic inflammation that limit mobility and cause pain.

The instruments amplify what the provider can feel in the tissue — areas of fibrosis, adhesion, and restriction become more detectable through the tool than with hands alone. This enhanced sensitivity allows for precise treatment of even deep tissue layers that are difficult to access manually.

How IASTM Works

When soft tissue is injured — whether from acute trauma, repetitive strain, or chronic postural stress — the body repairs the damage with collagen fibers laid down in a disorganized pattern. This scar tissue is weaker, less elastic, and more prone to re-injury than healthy tissue. Over time, layers of disorganized collagen accumulate and form adhesions that restrict movement and irritate surrounding structures.

IASTM instruments are stroked along the skin over the affected tissue with controlled pressure and speed. This mechanical stimulation creates a localized inflammatory response that restarts the body’s healing cascade. The treatment breaks up existing adhesions, stimulates the production of new, properly aligned collagen fibers, and increases blood flow to the area. The result is healthier, more functional tissue.

You may notice mild redness (petechiae) at the treatment site — this is a normal vascular response and typically fades within 24-48 hours.

What to Expect During Treatment

An IASTM session typically lasts 10-20 minutes as part of a comprehensive treatment visit. Your provider will first warm the target tissue through light movement or manual therapy, then apply the instrument using specific strokes, angles, and pressure levels based on the tissue being treated.

You may feel a scraping or pulling sensation during treatment. While it can be uncomfortable over areas of dense adhesion, the pressure is always adjusted to your tolerance. Mild bruising or redness at the treatment site is normal and resolves within a few days.

After IASTM, your provider will typically guide you through stretching and strengthening exercises to reinforce the tissue changes achieved during treatment. This combination of breaking down dysfunctional tissue and building healthy tissue is what produces lasting results.
